Bill;

You only need to look as far as your storm door (or store-bought aluminum  
storm doors) for a model.
They come with a "frame" that puts the door outside of the plane of the  
wall an amount that is the thickness of the door and moves the pivot point  
of the hinge to outside of the outside face of the door.

I've done the same with storm doors that I've made out of 2x6 lumber,  
making a frame out of 2x4 lumber that clips to the outside of the opening.

I suspect that if you were to Google "Walk-in refrigerator plans" you  
should get some hits on how they configure the doors for the walk-in  
refrigerators that are typically made by the builder for small  
restaurants, neighbourhood grocers  etc.
